Florida Residents: Must be a resident and U.S. Citizen (or permanent resident for 6+ months).

Age Requirements: Available for adults age 19–64.

Family Coverage: You can cover yourself, your spouse, and your children (up to age 22).

Effective Dates: Start on the 1st or 15th. Benefit is available once per year, beginning 90 days after the policy takes effect.
